Assisted Living or Residential Habilitation Direct care staff provide daily care and training in the areas of communication, personal care, daily living task, recreation and community accessibility.
Skilled Nursing - The nursing staff coordinate medical services and treatment, administer medication and treatments as ordered and provide guidance and training to promote a healthy lifestyle.
Case Management - The Case Manager coordinates and monitors individualized plans of care. The case manager also assists with accessing community services and activities.
Behavior Communication - The Behavior Communication Specialist assist individuals in communicating their wants and needs. Behaviors viewed as communication are managed through communication strategies.
Intervenor Services - An intervenor provides one on one assistance and guidance to the participant in accessing community resources and activities.
Specialized Medical - Through the waiver, financial assistance is provided for medications, medical equipment and dental treatment.
Environmental Accessibility - Through the waiver, financial assistance is provided for equipment or modifications necessary to promote independent functioning.
Orientation and Mobility Services - The Case Manager coordinates with a mobility specialist for evaluations and training that allow participants to safely ambulate around their environment.
Physical Therapy and Occupational Therapy - The nursing staff coordinates services with Licensed Physical Therapist and Occupational Therapist.
